What is Pradaxa?

Pradaxa (generic: dabigatran) is a blood thinning medication intended to prevent stroke in patients who suffer from an abnormal heart rhythm called atrial fibrillation.

Pradaxa was developed by Boehringer Ingelheim Pharmaceuticals Inc., and received FDA approval in 2010.

People with atrial fibrillation, an abnormal heart rhythm, can experience blood clots, because blood tends to pool inside the heart. Blood clots can be deadly if they travel through the blood stream and become lodged in the brain, causing a stroke.

Some patients and doctors reported that it sometimes had too much of an effect on clotting, leading to hemorrhages. Other reports indicated that at times it did not have enough of an effect, resulting in pulmonary embolism or deep vein thrombosis.

Pradaxa Mechanism of Action

Pradaxa is a type of blood-thinner called a “direct thrombin inhibitor,” which inhibits the blood enzyme thrombin, which is necessary for clotting. In some cases, people who take Pradaxa may experience serious internal bleeding because the blood becomes too thin and cannot coagulate.

Pradaxa Has No Antidote

Pradaxa has no antidote. It is marketed as a superior alternative to warfarin (coumadin). The blood-thinning effects of warfarin can be reversed by administering Vitamin K or fresh frozen plasma. Pradaxa has no similar safety mechanism.

FDA Warns of Risk

The FDA warns that you may have a higher risk of bleeding if you take Pradaxa and you:

  • are over 75 years old
  • have kidney problems
  • have recent stomach or intestinal bleeding or you have a stomach ulcer

You may be at increased risk of serious bleeding if you take other medicines that increase your risk of bleeding, including:

  • aspirin or aspirin containing products
  • chronic use of non-steroidal anti-inflammatory drugs (NSAIDs)
  • warfarin sodium (Coumadin®, Jantoven®)
  • heparin
  • clopidogrel (Plavix®)
What is Atrial Fibrillation

Atrial fibrillation is a heart rhythm disorder caused by a malfunctioning electrical conduction system within the heart. It is an irregular rapid heart rate that begins in the smaller 2 upper heart chambers, known as the atria, that begins to contract quickly and in an irregular fashion.

As electrical signals leave the atria and enter the ventricles, they too begin to contract quickly and in an irregular fashion. This irregular and rapid heart rate is unable to pump the blood through the heart and out to the body. Blood can pool in the heart and the lower extremities allowing clots to form.

These clots dislodge and travel to the heart, lungs, and brain leading to a stroke.

Its estimated 2.3 million Americans have atrial fibrillation and the number is expected to increase to 5.6 million by 2050.

Atrial fibrillation increased the risk of stroke 5 times, and is an underlying condition in 15% of all US strokes. Atrial fibrillation related strokes are twice as likely to be fatal or severely disabling when compared to strokes from other causes.

Pradaxa Bleeding Lawsuit 2014 Settlement Boehringer Ingelheim agreed to pay more than $650 million in 2014 to settle thousands of Pradaxa lawsuits. Now the drug maker will face a new round of cases alleging that users suffered severe and often fatal bleeding problems after using the controversial new anticoagulant. These new Pradaxa injury lawsuits are filed in Connecticut State Court. The complaints raise similar allegations, claiming that Boehringer Ingelheim has continued to provide inadequate warnings about the risk of bleeding from Pradaxa side effects. There has been no changes to the warning label. After several years of centralized proceedings in a federal multidistrict litigation (MDL), Boehringer Ingelheim agreed to settle about 4,000 product liability lawsuits in May 2014, paying an average of about $150,000 per case. The settlement deal was reached just as the first cases were set to go to trial, addressed all Pradaxa injury cases pending at the time. The second wave of Pradaxa litigation is being pushed at the state court level, and thousands of similar Xarelto lawsuits are also being filed on behalf of individuals who suffered uncontrollable bleeding after using that anticoagulant.
