The U.S. Food and Drug Administration (FDA) approved Xarelto in July 2011 for a limited group of people, getting knee or hip replacement surgery, because they are at a higher risk of developing blood clots that could lead to a stroke, deep vein thrombosis or pulmonary embolism.
In November 2011, Xarelto was approved to prevent stroke in people with abnormal heart rhythm, also known as non-valvular atrial fibrillation. Atrial fibrillation leads to heart blood clots, which travels to the brain, causing a stroke.
Despite all the advertisements, Xarelto does cause more gastrointestinal bleeding than the standard therapy for blood clot prevention, Coumadin but more importantly there is no antidote for uncontrollable bleeding.
Coumadin bleeding side effects can be reversed with Vitamin K and blood transfusions.
Xarelto is known as a “direct thrombin inhibitor,” Xarelto was designed to compete with Boehringer Ingelheim’s Pradaxa.
Both drugs were released to replace the blood thinner warfarin, also known as Coumadin, which has dominated the atrial fibrillation blood thinner market for several decades.
Since it was introduced, hundreds of adverse event reports have been submitted to the FDA, noting a link between Xarelto and bleeding injuries or death.

An April 2011 medical study involving 8,000 people compared Xarelto to Lovenox (an injectable blood thinner) in patients hospitalized for heart failure, infectious disease and breathing difficulty medical conditions.
The study compared Xarelto to Lovenox used over 10 days, and found the two medications equally effective. Patients taking Xarelto for 35 days had significantly higher rates of bleeding, they were more than twice as likely to experience bleeding compared to Lovenox.Xarelto “ROCKET” Safety Study
The biggest clinical trial of Xarelto evaluated more than 14,000 patients, and compared Xarelto to warfarin (older oral blood thinner drug). The study found that Xarelto was no more effective than warfarin, and the side effect risks were similar.
The most common side effect was bleeding, with rates similar to warfarin. It caused less bleeding in the brain and more bleeding in the stomach and intestines.
The safety and effectiveness of Xarelto has been questioned, after a report in the New England Journal of Medicine found that the use of multiple statistical analyses “muddied the waters.”
FDA reviewers who examined the data found that the drug-makers failed to administer warfarin in an optimal way, which may have skewed the data.
